HIV Care Coordination Program
The Proactive Community Services care coordination model utilizes an interdisciplinary team-based client approach. We combine primary care, medical case management, housing navigation/technician, ADAP (AIDS Drug Assistance Program) 340B prescription service with pharmacy assistance and medical adherence, and community health outreach with structured health education to promote HIV care continuum engagement and viral suppression.

This site contains HIV prevention messages that may not be appropriate for all audiences. Since HIV infection is spread primarily through sexual practices or by sharing needles, prevention messages and programs may address these topics.
